For Referring Veterinarians

Below is a patient referral form for a patient tx plan from a DVM for transfer to NB Animal Urgent Care for overnight care with an LVT/RVT. Referring veterinarians, please fill out the DVM referral form. If you have any questions, call (830) 420-6719. Se habla español.

Please make sure you have a signed authorization waiver from your client acknowledging there will not be a DVM on staff between 11pm and time of discharge by 8am the next morning. All care delivered to their pet is followed based on the information provided by their rDVM

Patient's care will be documented with all inputs and outputs (e/d/u/d/v) and sent back to you by 8am the next day!
